Job Description:
Community based residential facilities and home visits within a 25-mile radius FTE:

9 Palliative Care and 0.
1 Hospice Monday - Friday, flexible for 8 or 10 hour workdays 4-to-5-day work week Flexibility with scheduling to maintain a work life balance No call, weekends or holidays Three to five patients per day Enjoy seeing primarily geriatric patients RN or NP experience in palliative or hospice preferred

Qualifications:
Licensed Nurse Practitioner in the state of Wisconsin.
